I normally use this:
Just generates a fake US profile with associated address, credit card number, occupation, moneygram codes, etc.
Site also has some good tools for other stuff - generating parcel tracking numbers, vehicle identification numbers, insurance numbers, even stuff for other countries (UK National insurance numbers for example).
Has a cool smiley generator too, which seems to completely at odds with the rest of the site.
